I’m Kaylee Grahnert, MSN, FNP-BC I am a board-certified Family Nurse Practitioner with a Master of Science in Nursing from Walden University. My background as a Registered Nurse has provided me with extensive experience managing patients with complex chronic conditions, acute illnesses, and a wide range of healthcare needs.
Throughout my nursing career, I have developed a strong foundation in comprehensive patient assessments, disease process management, recognizing and responding to acute changes in health status, and coordinating individualized care. My experience in memory care has further strengthened my clinical knowledge in caring for individuals with cognitive impairment, dementia, and complex medical needs, while emphasizing dignity, compassion, and quality of life.
These experiences have shaped my approach to healthcare, reinforcing the importance of prevention, early intervention, patient education, and building meaningful relationships with those I care for.
